Job Description

St. James Rehabilitation and Healthcare Center in Long Island seeks a Certified Occupational Therapy Assistant to provide hands-on therapy services in a skilled nursing and rehab setting. The COTA will implement treatment plans under OT supervision, focusing on functional gains, safety, and independence in ADLs for short-term rehab and long-term care residents. Responsibilities include treatments, documentation, family education, and collaboration with an interdisciplinary team in a compassionate, resident-centered environment that supports professional growth.

Responsibilities

  • Provide therapeutic treatments under the supervision of an Occupational Therapist
  • Implement and progress individualized treatment plans to improve function and independence
  • Assist with resident assessments, goal setting, and care planning
  • Document treatments, patient responses, and progress accurately and timely
  • Educate residents and families on exercises, adaptive techniques, and safety
  • Collaborate with nursing, rehab, and interdisciplinary team members
  • Maintain treatment areas, equipment, and infection control standards
  • Support discharge planning and recommendations for adaptive equipment
