Job Description
Solaris HealthCare seeks a compassionate Physical Therapist to provide evidence-based rehabilitation services in our skilled nursing and long-term care center. You will evaluate residents' mobility and functional needs, develop personalized treatment plans, and deliver therapeutic exercises, gait training, balance, and strengthening programs. Working within a supportive, patient-centered, team environment, you will collaborate closely with nursing and rehab staff, maintain accurate EMR documentation, educate residents and families, and help promote dignity, safety, and improved quality of life for every resident.
Responsibilities
- Evaluate residents' functional mobility, strength, balance, and safety
- Develop and implement individualized, evidence-based treatment plans
- Provide therapeutic exercises, gait training, and manual therapy
- Document assessments, goals, and progress in EMR accurately and timely
- Collaborate with nursing, rehab, and medical teams on care plans
- Educate residents and families on home programs, safety, and equipment use
- Participate in care conferences and quality-improvement initiatives
- Support a dignified, compassionate, and patient-centered environment
