Job Description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to deliver comprehensive clinical assessments in patients' homes. You will evaluate complex and senior populations, review medical histories and medications, and identify care gaps using digital tools and data insights. In collaboration with primary care and interdisciplinary teams, you will develop care plans that support value-based care and reduce avoidable hospitalizations. Responsibilities
- Conduct comprehensive in-home health assessments for medically complex and senior patients.
- Review medical history, medications, and social determinants to identify risks and care gaps.
- Develop evidence-based care plans and coordinate with primary care providers and care teams.
- Document encounters accurately using Signify Health's digital tools and EMR systems.
- Order and interpret diagnostic tests within scope of practice and applicable regulations.
- Educate patients and caregivers on conditions, medications, and preventive care.
- Collaborate with interdisciplinary teams to support value-based care initiatives.
- Adhere to clinical quality, compliance, and safety standards in home settings.
- Use data-driven insights to improve outcomes and reduce avoidable hospitalizations.
- Participate in ongoing training, quality improvement, and performance feedback programs.
