In-Home Nurse Practitioner or Physician Assistant (Full Time)

Signify Health is hiring an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive clinical assessments in members' homes across the Dallas area. You will evaluate medical history, conduct physical exams, close care gaps, and document findings in our technology platform to support value-based care. Collaborating with interdisciplinary teams, you'll identify social and clinical needs, educate patients, and coordinate follow-up care. Responsibilities

• Perform comprehensive in-home health assessments for members in the Dallas area
• Review medical history, medications, and social determinants of health
• Conduct physical exams and document findings in the EHR and Signify platforms
• Identify and close care gaps, supporting quality and value-based care metrics
• Develop clinical impressions and recommendations for follow-up care
• Educate patients and caregivers on conditions, medications, and preventive care
• Coordinate with health plans, PCPs, and interdisciplinary teams for care continuity
• Use mobile technology and analytics tools to guide assessments and documentation
• Maintain compliance with clinical, regulatory, and documentation standards
• Contribute to a mission-driven, collaborative culture focused on improving outcomes
