In-Home Nurse Practitioner or Physician Assistant (Full Time)

Signify Health is hiring an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health assessments for vulnerable patients in their homes. Responsibilities

• Perform comprehensive in-home health assessments for assigned patients
• Review medical history, medications, and social determinants of health
• Identify care gaps and risk factors, escalating concerns as needed
• Document encounters accurately in electronic systems and tools
• Collaborate with interdisciplinary teams to support care plans
• Educate patients and caregivers on conditions, medications, and self-care
• Coordinate follow-up services and referrals as appropriate
