NURSE PRACTITIONER

Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive, in-home clinical evaluations for diverse, often vulnerable patients. You will review medical histories, complete physical exams, and assess social determinants of health, leveraging Signify's advanced technology and data to identify care gaps and improve outcomes. Working within a collaborative, value-based care model, you will develop care recommendations, educate patients and caregivers, and coordinate with interdisciplinary teams. Responsibilities

• Conduct comprehensive in-home health evaluations for diverse and often vulnerable patient populations.
• Review medical histories, medications, and social determinants of health to identify care gaps and risks.
• Perform physical exams, order and interpret diagnostic tests within scope of practice.
• Document findings accurately in electronic health records using Signify Health's technology platform.
• Develop and communicate individualized care plans and recommendations to patients and care teams.
• Collaborate with physicians, care coordinators, and health plan partners to support value-based care goals.
• Educate patients and caregivers on condition management, prevention, and community resources.
• Ensure compliance with clinical guidelines, regulatory standards, and payer requirements.
• Use data-driven insights to improve quality metrics and patient outcomes.
• Participate in ongoing training, quality improvement, and professional development initiatives.
