Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive clinical evaluations in patients' homes. Responsibilities
- Conduct comprehensive in-home health assessments for diverse, often vulnerable patient populations.
- Review medical histories, medications, and social determinants to identify gaps in care and risk factors.
- Develop evidence-based care recommendations and coordinate with primary care and specialist providers.
- Document visits accurately in Signify Health's technology platform and ensure regulatory compliance.
- Collaborate with interdisciplinary teams using data and analytics to improve patient outcomes.
- Educate patients and caregivers on disease management, preventive care, and community resources.
- Escalate urgent clinical concerns and facilitate appropriate follow-up care or referrals.
- Contribute to quality improvement initiatives and value-based care programs.
- Maintain patient safety, infection control, and professional standards in home environments.
- Participate in ongoing training to stay current with guidelines, technology, and best practices.
Required Skills
- Family or Adult-Gerontology Nurse Practitioner or Physician Assistant clinical practice
- Comprehensive geriatric and adult primary care assessment
- Chronic disease management (e.g., CHF, COPD, diabetes)
- Medication reconciliation and polypharmacy review
- Use of EMR/EHR and mobile clinical documentation tools
- Knowledge of value-based care and risk adjustment (HCC coding)
- Care coordination and interdisciplinary collaboration
- Patient education and motivational interviewing
- Home health safety assessment and care planning
- Regulatory compliance and documentation standards (CMS, HIPAA)
