OCCUPATIONAL THERAPIST

LifePoint Health is seeking a dedicated Occupational Therapist to join our Healthcare & Medical Services team. In this role, you will assess, plan, and implement individualized therapy programs to help patients regain function, independence, and quality of life. You will collaborate closely with physicians, nurses, and other therapists to deliver patient-focused, evidence-based care. Responsibilities

• Perform comprehensive evaluations of patients' physical, cognitive, and psychosocial function related to daily activities and functional mobility.
• Develop and implement individualized, evidence-based treatment plans to improve independence with self-care, home management, work, and leisure activities.
• Provide therapeutic interventions including ADL training, therapeutic exercise, neuromuscular re-education, cognitive and perceptual retraining, and functional mobility and balance training.
• Recommend, train, and educate patients and caregivers on the use of adaptive equipment, assistive devices, and environmental modifications to support safety and independence.
• Document assessments, treatment sessions, progress notes, and discharge summaries accurately and timely within the electronic medical record system.
• Collaborate with physicians, nurses, physical therapists, speech-language pathologists, case managers, and other team members to coordinate care and participate in interdisciplinary care conferences.
• Contribute to discharge planning by assessing ongoing therapy needs, recommending follow-up services, and providing home exercise programs and education to patients and families.
• Adhere to all hospital policies, regulatory standards, and infection control and safety guidelines to maintain a safe and effective care environment.
