Job Description
Essentia Health seeks a full-time Physical Therapist to provide evidence-based, patient-centered rehabilitation services in an integrated health system. Responsibilities include evaluating patients with musculoskeletal, neurological, and post-surgical conditions; developing individualized treatment plans; delivering therapeutic exercises, manual therapy, and modalities; and educating patients and families on home programs and prevention. Collaborate with an interprofessional care team, document in the EMR, and contribute to quality and process improvement. Responsibilities
- Evaluate patients and perform comprehensive physical therapy assessments
- Develop and implement individualized treatment plans and goals
- Provide therapeutic exercise, manual therapy, and therapeutic modalities
- Educate patients and families on home exercise programs and injury prevention
- Collaborate with physicians, nurses, and other healthcare professionals
- Document evaluations, treatments, and progress notes in the EMRMonitor patient progress and adjust plans of care as needed
- Participate in quality improvement and evidence-based practice initiatives
- Ensure compliance with regulatory, safety, and practice standards
- Contribute to a compassionate, patient-centered care environment
