Case Manager RN

Job Description

Providence Health and Services seeks a Case Manager RN to coordinate and advocate for patient care across the continuum. In this mission-driven, patient-centered role, you will conduct clinical assessments, develop and update care plans, facilitate safe transitions of care, and collaborate closely with physicians, interdisciplinary teams, patients, and families. The Case Manager RN promotes appropriate utilization of services, supports discharge planning, reduces readmissions, and connects patients with community resources while upholding Providence's values of compassion, respect, and social justice.

Responsibilities

  • Perform comprehensive nursing and psychosocial assessments to identify patient needs and risks
  • Develop, implement, and regularly update individualized care and discharge plans
  • Coordinate services and transitions of care across hospital, home, and community settings
  • Collaborate with physicians, nurses, social workers, and other team members to optimize outcomes
  • Monitor utilization of services and promote evidence-based, cost-effective care
  • Educate patients and families on care plans, medications, and community resources
  • Identify and address barriers to care, including social determinants of health
  • Document assessments, plans, and outcomes accurately in the EHR and per regulatory standards
  • Support quality, safety, and readmission reduction initiatives
  • Advocate for patients while upholding Providence's mission, values, and policies
