Job Description
Carle Health is seeking a compassionate Social Worker to deliver clinical social work services in a community-focused healthcare setting. In this role, you will conduct psychosocial assessments, provide counseling, and develop care plans to improve patient outcomes. You will coordinate discharge planning, connect patients and families with community resources, and advocate for access to needed services. Responsibilities
- Provide psychosocial assessments, counseling, and care planning for patients and families
