LifeBridge Health is seeking a PRN Occupational Therapist to deliver patient-centered, evidence-based care across our continuum of services. In this flexible role, you will perform comprehensive evaluations, design and implement individualized treatment plans, and focus on improving functional independence and quality of life. Responsibilities
- Provide evidence-based occupational therapy evaluations and treatments for diverse patient populations across the care continuum
- Develop, implement, and adjust individualized treatment plans to improve functional independence and quality of life
- Collaborate with physicians, nurses, rehab staff, and families to coordinate comprehensive patient-centered care
- Educate patients and caregivers on adaptive techniques, home programs, and safe use of assistive devices
- Document assessments, treatment sessions, progress notes, and discharge summaries in the EMR accurately and timely
- Participate in interdisciplinary team meetings and contribute to care planning and discharge recommendations
- Adhere to regulatory, ethical, and documentation standards and Life

- Support quality improvement, patient safety, and evidence-based practice initiatives within the rehab team
Required Skills
- Occupational therapy evaluation and treatment planning
- Knowledge of functional and ADL retraining techniques
- Use of adaptive equipment and assistive technology
