Job Description
Adventist Health is seeking a compassionate Physical Therapist Assistant to deliver in-home rehabilitation services to patients across our community. In this faith-based, mission-driven role, you will provide treatments under the direction of a licensed Physical Therapist, focusing on mobility, strength, and functional independence. Responsibilities include implementing care plans, documenting progress, educating patients and caregivers, and ensuring safety in the home environment. Responsibilities
- Provide in-home physical therapy interventions under the supervision of a licensed PTFollow individualized care plans and document patient progress in EMR systems
- Assist patients with mobility, strength, balance, and functional training
- Educate patients and caregivers on home exercise programs and safety strategies
- Communicate changes in patient status to the PT and care team
- Travel to patient homes within assigned service area
- Ensure compliance with clinical, safety, and regulatory standards
- Contribute to a mission-driven, faith-based culture of compassionate care
