Home Health Physical Therapist

WSi Healthcare Personnel seeks a compassionate Home Health Physical Therapist to deliver one-on-one rehab services in patients' homes. In this role, you'll evaluate functional mobility, develop evidence-based care plans, and provide therapeutic interventions to help patients regain independence. You'll collaborate with an interdisciplinary team, educate patients and caregivers, and document care in EMR. Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes
• Develop and implement individualized, evidence-based treatment plans
• Provide therapeutic exercises, gait and balance training, and functional mobility interventions
• Educate patients and caregivers on home exercise programs and safety
• Collaborate with physicians, nurses, and other care team members
• Document assessments, treatments, and outcomes accurately in EMRMonitor patient progress and adjust plans of care as needed
• Ensure compliance with home health, payer, and regulatory standards
• Communicate schedule and visit updates with staffing coordinators
• Participate in ongoing training and quality improvement initiatives
