University Hospitals Rehabilitation Hospital in Beachwood, Ohio is seeking a Registered Nurse to join our inpatient acute rehabilitation team. In this role, you will deliver evidence-based, patient-centered care to adults recovering from stroke, brain and spinal cord injuries, and other complex conditions. You will collaborate closely with an interdisciplinary team to develop and implement individualized care plans, monitor patient progress, educate patients and families, and support safe transitions. Responsibilities
- Provide comprehensive, evidence-based nursing care to adult patients in an inpatient acute rehabilitation setting.
- Perform ongoing patient assessments and develop, implement, and evaluate individualized nursing care plans.
- Administer medications, treatments, and interventions safely and accurately, monitoring for and documenting patient responses.
- Collaborate closely with physicians, therapists, case managers, and other team members in an interdisciplinary care model.
- Support and reinforce therapy goals by assisting patients with mobility, self-care, and functional activities throughout the day.
- Provide patient and family education on diagnoses, medications, safety, rehabilitation goals, and discharge plans.
- Monitor for changes in patient condition, respond promptly to clinical concerns, and escalate issues according to hospital protocols.
- Maintain accurate, timely documentation in the electronic health record, including assessments, interventions, and outcomes.
- Promote a safe environment by following fall prevention, infection control, and safe patient-handling practices.
- Participate in interdisciplinary team conferences, quality improvement initiatives, and ongoing professional development activities.
