Registered Nurse

Lancaster Rehabilitation Hospital is seeking a compassionate Registered Nurse to join our Healthcare & Medical Services department. In this inpatient rehabilitation setting, you'll provide individualized, evidence-based care to patients recovering from stroke, brain and spinal cord injuries, neurological conditions, orthopedic surgeries, and complex medical issues. Responsibilities

• Deliver safe, evidence-based nursing care to adult rehabilitation inpatients recovering from stroke, brain and spinal cord injury, neurological conditions, orthopedic surgeries, and complex medical conditions.
• Conduct comprehensive and ongoing patient assessments, including neurological, musculoskeletal, cardiopulmonary, and pain assessments, to inform individualized care plans.
• Administer medications, IV therapies, and treatments accurately and safely, monitoring for effectiveness and adverse reactions.
• Implement and update individualized nursing care plans in collaboration with physicians, therapists, case managers, and other members of the interdisciplinary rehabilitation team.
• Support intensive rehabilitation goals by coordinating nursing care with physical, occupational, and speech therapy schedules and treatment plans.
• Provide specialized rehabilitation-focused nursing interventions, including mobility support, positioning, skin integrity management, bowel and bladder programs, and safety interventions.
• Perform and document wound care, surgical site assessments, and pressure injury prevention measures according to best practices and facility protocols.
• Monitor vital signs, intake and output, lab results, and clinical status, promptly recognizing and reporting changes in patient condition.
• Educate patients and families on disease processes, medications, safety strategies, adaptive techniques, and home care to promote independence and successful transitions of care.
• Maintain thorough, accurate, and timely documentation in the electronic health record (EHR) in accordance with regulatory, accreditation, and hospital standards.
• Participate actively in interdisciplinary team meetings, care conferences, and goal-setting to optimize functional outcomes and discharge planning.
• Promote a safe environment by adhering to infection prevention, fall prevention, and hospital safety policies and procedures.
• Provide compassionate, patient-centered support that respects patient dignity, cultural differences, and individual goals and preferences.
• Precept, mentor, and support new staff or nursing students as requested, contributing to a culture of continuous learning and clinical excellence.
• Engage in ongoing professional development, skills training, and competency validation related to rehabilitation nursing and complex medical care.
