Centra Rehabilitation Hospital, part of Centra Health, is seeking a compassionate Registered Nurse to join our Healthcare & Medical Services team. Responsibilities
- Provide direct, evidence-based nursing care to adult and geriatric patients in an inpatient rehabilitation setting.
- Perform comprehensive admission and ongoing assessments, including neurological, musculoskeletal, and functional status evaluations.
- Develop, implement, and regularly update individualized plans of care in collaboration with the interdisciplinary rehab team.
- Administer medications and treatments safely and accurately, monitoring for therapeutic response and adverse reactions.
- Assist patients with mobility, transfers, ADLs, and use of assistive devices, reinforcing therapy goals between therapy sessions.
- Monitor and document vital signs, pain levels, intake and output, and changes in condition, escalating concerns promptly.
- Educate patients and families on diagnosis, medications, self-care strategies, and discharge plans to support safe transition home or to the next level of care.
- Participate actively in interdisciplinary team conferences, contributing nursing assessments and progress updates to optimize outcomes.
- Implement fall prevention, skin integrity, and infection prevention protocols consistent with hospital and regulatory standards.
- Use electronic health records to document assessments, interventions, and patient responses in a timely, accurate, and compliant manner.
