Job Description
VNS Health is seeking a dedicated Licensed Practical Nurse to deliver compassionate home and community-based care throughout New York City. The LPN will provide direct patient care, administer medications, monitor health status, and support individualized care plans under the supervision of an RN. Responsibilities
- Provide compassionate nursing care to patients in their homes and community settings
- Administer medications and treatments as prescribed
- Monitor and document vital signs, patient status, and responses to care
- Collaborate with RNs, physicians, therapists, and caregivers to implement care plans
- Educate patients and families on disease management, medications, and home care techniques
- Ensure compliance with infection control and safety protocols
- Accurately maintain electronic health records and required documentation
- Communicate changes in patient condition to the care team promptly
