Job Description

Signify Health is seeking a mission-driven Physician Assistant to perform comprehensive in-home health evaluations for diverse, often vulnerable, patient populations. You will conduct detailed histories, physicals, and medication reviews; identify care gaps; and document findings using Signify Health's digital tools to support value-based care. Partnering with physicians, care coordinators, and health plan partners, you'll help shape individualized care plans and educate patients on chronic disease management. Responsibilities

  • Perform comprehensive in-home health assessments for diverse, often vulnerable, patient populations.
  • Review medical histories, medications, and social determinants to identify care gaps and risks.
  • Document findings accurately in Signify Health's digital platforms and EHR systems.
  • Collaborate with physicians, nurses, and care coordinators to develop and refine care plans.
  • Educate patients and caregivers on chronic disease management, prevention, and follow-up care.
  • Use evidence-based guidelines and data insights to support value-based care outcomes.
  • Communicate assessment results to health plans and provider partners in a timely manner.
  • Adhere to clinical quality, compliance, and documentation standards and protocols.
  • Participate in continuous quality improvement, training, and performance feedback.
  • Contribute to a collaborative, mission-driven culture focused on improving patient outcomes.
