Responsibilities
- Perform comprehensive in-home health assessments for diverse and often vulnerable patient populations
- Review medical history, medications, and social determinants of health to identify care gaps
- Document findings accurately in electronic health record and digital assessment tools
- Collaborate with physicians, nurses, and care coordinators to support value-based care plans
- Provide patient education on chronic disease management, medications, and preventive care
- Order and interpret basic diagnostic tests within scope of practice
- Communicate risk factors and care opportunities to health plan and provider partners
- Follow evidence-based guidelines and organizational protocols to ensure quality and compliance
- Maintain timely, accurate, and secure documentation aligned with regulatory standards
- Participate in quality improvement initiatives and use data insights to enhance patient outcomes
