Signify Health is seeking a Physician Assistant to deliver comprehensive in-home evaluations and value-based care for vulnerable populations. Responsibilities
- Conduct comprehensive in-home health assessments for diverse and often vulnerable patient populations.
- Review medical histories, medications, and social determinants of health to identify care gaps and risk factors.
- Develop evidence-based care plans and collaborate with physicians, nurses, and care coordinators.
- Leverage Signify Health's technology and analytics tools to document findings and support value-based care.
- Educate patients and caregivers on chronic disease management, preventive care, and community resources.
- Coordinate follow-up services and referrals to providers, specialists, and community programs.
- Ensure accurate, timely, and compliant documentation in the electronic health record.
- Contribute to quality improvement initiatives and use data to drive better patient outcomes.
- Maintain up-to-date knowledge of clinical guidelines, coding requirements, and regulatory standards.
- Participate in interdisciplinary meetings and support a collaborative, mission-focused culture.
