RN Intake Coordinator

Adventist Health seeks an RN Intake Coordinator to guide patients into our faith-based, whole-person care system. In this role, you'll perform clinical intake assessments, triage needs, and coordinate timely access to hospital, clinic, and home care services. You will collaborate with physicians, nurses, and care teams to streamline admissions, referrals, and transitions of care while educating patients and families on next steps. Working in a mission-driven, compassionate environment, you'll use your clinical judgment, EMR skills, and communication abilities to ensure safe, efficient, and dignified patient access to care.

Responsibilities

• Conduct RN-level intake assessments for new and returning patients
• Triage patient needs and prioritize based on clinical urgency
• Coordinate admissions, referrals, and transitions of care
• Collaborate with physicians, nurses, and care teams to develop care plans
• Educate patients and families on services, next steps, and resources
• Document assessments, care coordination, and communications in the EMREnsure compliance with clinical, regulatory, and organizational standards
• Support Adventist Health's mission of faith-based, whole-person care
