Occupational Therapy Assistant

Adventist Health is seeking a dedicated Occupational Therapy Assistant to provide compassionate home health services focused on whole-person care-mind, body, and spirit. In this field-based role, you will deliver occupational therapy interventions under the supervision of an Occupational Therapist, helping patients safely maximize independence in their homes. Responsibilities include implementing care plans, providing ADL training, educating patients and caregivers, documenting in the EMR, and collaborating with an interdisciplinary, faith-based team. Responsibilities

• Provide home-based occupational therapy interventions under the supervision of an Occupational Therapist
• Assist in developing and implementing individualized care plans to promote independence and safety
• Conduct patient visits in the home, monitoring progress and documenting outcomes in the EMREducate patients and caregivers on adaptive techniques, equipment use, and home safety
• Collaborate with interdisciplinary team members to coordinate holistic, faith-based care
• Adhere to clinical, regulatory, and documentation standards for home health services
• Participate in case conferences and contribute to care planning
• Support patient spiritual and emotional needs consistent with Adventist Health values
• Maintain accurate, timely, and compliant clinical records
• Demonstrate commitment to professional growth and quality improvement
