This role focuses on outreach and support to communities affected by Hepatitis C Virus (HCV), enhancing access to essential medical and social services. Utilizing a mobile healthcare model, this position plays a crucial role in our mission to reduce HCV spread and improving patient outcomes through personalized care and education.

Responsibilities

  • Conduct field-based health education on HCV prevention and treatment.
  • Provide support and navigation services to patients accessing HCV care.
  • Collaborate with healthcare providers to coordinate patient care plans.
  • Collect and report data on patient outcomes and program effectiveness.
  • Participate in community outreach events and health screenings.
