LICENSED PRACTICAL NURSE

CenterWell Home Health seeks a compassionate Home Health Licensed Practical Nurse to deliver skilled, patient-centered care in the home. The LPN will perform assessments, administer medications and treatments, monitor chronic conditions, and reinforce care plans under RN/physician direction. You will educate patients and caregivers, promote safety and independence, document care accurately, and collaborate with an interdisciplinary team. Responsibilities

• Provide direct skilled nursing care to patients in their homes per care plan and physician orders
• Perform patient assessments, vital signs monitoring, and ongoing condition evaluation
• Administer medications, wound care, injections, and other prescribed treatments
• Educate patients and caregivers on disease management, medications, and home safety
• Document all visits and interventions accurately in the electronic health record
• Communicate changes in patient status promptly to the RN case manager and physician
• Coordinate with interdisciplinary team members to support patient goals and discharge planning
• Promote patient autonomy, safety, and adherence to care plans during each visit
