Home Health Physical Therapist

CenterWell Home Health, part of Humana Inc., seeks a Home Health Physical Therapist-PRN to deliver compassionate, in-home therapy for seniors and adults with chronic conditions. In this PRN role, you will perform evaluations, create personalized care plans, and provide evidence-based treatments to improve mobility, safety, and independence at home. You'll educate patients and caregivers, document care in EMRs, and collaborate with an interdisciplinary team. Responsibilities

• Provide in-home physical therapy evaluations and develop individualized care plans for adult and senior patients
• Deliver evidence-based therapeutic exercises, gait training, and mobility interventions to improve function and safety
• Educate patients and caregivers on home exercise programs, fall prevention, and use of assistive devices
• Collaborate with an interdisciplinary team including nurses, therapists, and physicians to coordinate care
• Document visits, progress notes, and care plans accurately and timely in electronic medical records
• Adjust treatment plans based on patient progress and clinical judgment to support independence at home
• Communicate effectively with patients, families, and care team members regarding goals and outcomes
