Home Health Physical Therapist

CenterWell Home Health seeks a compassionate Home Health Physical Therapist to deliver one-on-one rehabilitative care in patients' homes. In this role, you'll evaluate functional status, develop and adjust individualized treatment plans, and provide evidence-based therapeutic interventions to improve mobility, safety, and independence. You'll educate patients and caregivers on exercises, equipment, and fall prevention, while coordinating closely with nurses, physicians, and other clinicians. Responsibilities

• Perform comprehensive PT evaluations in patients' homes and establish individualized care plans
• Deliver evidence-based therapeutic interventions to improve strength, mobility, and balance
• Educate patients and caregivers on home exercise programs, safe transfers, and equipment use
• Monitor patient progress, adjust treatment plans, and document care in the EHR accurately and timely
• Collaborate with nurses, physicians, and other clinicians to coordinate holistic, patient-centered care
• Assess home safety risks and recommend modifications or equipment to reduce falls and injury
• Communicate status updates and recommendations to patients, families, and the care team
• Ensure compliance with regulatory, safety, and quality standards in all patient encounters
