Job Description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to deliver comprehensive evaluations for patients in their homes. You will perform detailed health assessments, review histories and medications, evaluate functional and social needs, and develop evidence-based care plans that support value-based care. Using EMR and data-driven insights, you'll identify care gaps, document thoroughly, and collaborate with interdisciplinary teams to improve outcomes for vulnerable populations. Responsibilities
- Perform comprehensive in-home health evaluations for diverse and vulnerable patient populations
- Assess medical history, medications, functional status, and social determinants of health
- Develop evidence-based care plans and communicate findings to primary care and care management teams
- Document visits accurately and efficiently in electronic medical records (EMR) systems
- Collaborate with interdisciplinary teams to support value-based care and care coordination
- Use data and clinical insights to identify care gaps and recommend interventions
- Educate patients and caregivers on condition management, medications, and preventive care
- Maintain compliance with clinical, regulatory, and documentation standards
- Participate in quality improvement initiatives and adopt best practices in home-based care
