Job Description
Ciena Healthcare is seeking a compassionate Registered Nurse to provide resident-centered care in our skilled nursing and rehabilitation centers. The RN will complete assessments, administer medications and treatments, coordinate care plans, and collaborate with an interdisciplinary team to support post-acute recovery, long-term care, and memory care residents. Responsibilities include supervising CNAs, monitoring changes in condition, documenting in the EMR, and communicating with families and providers. Responsibilities
- Provide direct nursing care to skilled nursing, rehab, and long-term care residents
- Complete comprehensive assessments and ongoing clinical evaluations
- Administer medications and treatments per provider orders and facility policies
- Develop, implement, and update individualized care plans with the care team
- Monitor resident status, identify changes in condition, and initiate appropriate interventions
- Document assessments, care, and outcomes accurately in the EMRCollaborate with physicians, therapists, social workers, and families to coordinate care
- Supervise and support CNAs and other support staff on assigned unit
- Educate residents and families on care plans, medications, and health management
- Follow infection control, safety, and regulatory standards in a 24/7 care setting
