In-Home Nurse Practitioner or Physician Assistant (Full Time)

Signify Health is seeking a full-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health assessments in members' homes. You will review medical histories, conduct physical exams, identify risk factors, and document findings in an EMR to support value-based care programs. Collaborating with interdisciplinary teams, you'll help close care gaps, educate patients and caregivers, and coordinate follow-up care. This role blends clinical autonomy, travel to patient homes, and technology-driven care to improve outcomes for vulnerable populations.

Responsibilities

• Perform comprehensive in-home health assessments for health plan members
• Review medical history, medications, and social determinants of health
• Conduct head-to-toe physical exams and identify risk factors and care gaps
• Document findings accurately and timely in EMR systems
• Provide patient and caregiver education on conditions, medications, and self-care
• Collaborate with interdisciplinary teams to support value-based care plans
• Recommend appropriate follow-up with primary care or specialists
• Ensure safety, professionalism, and privacy during home visits
• Use data and technology tools to inform clinical decision-making
• Travel within assigned territory to complete scheduled member visits
