In-Home Nurse Practitioner or Physician Assistant (Part Time)

Signify Health is seeking an in-home Nurse Practitioner or Physician Assistant to perform clinical evaluations for health plan members in their homes. You will complete comprehensive assessments, review medical histories, reconcile medications, and identify care gaps using Signify's technology platform. Collaborating with a multidisciplinary team, you'll document findings, educate patients, and support value-based care plans that improve outcomes for vulnerable populations while enjoying flexible, part-time work and a supportive, mission-driven culture.

Responsibilities

• Perform comprehensive in-home health assessments for health plan members.
• Review medical history, medications, and social determinants of health.
• Document findings accurately in Signify Health's technology platform/EHR.
• Identify care gaps and communicate recommendations to care teams and health plans.
• Educate patients and caregivers on conditions, medications, and preventive care.
• Collaborate with interdisciplinary teams to support value-based care plans.
• Use data and analytics tools to inform clinical decision-making.
• Follow evidence-based guidelines, regulatory requirements, and quality standards.
• Coordinate follow-up care or referrals as needed for high-risk patients.
• Maintain patient safety, professionalism, and cultural sensitivity in home settings.
