In-Home Traveler Nurse Practitioner or Physician Assistant

Signify Health seeks an In-Home Traveler Nurse Practitioner or Physician Assistant to perform comprehensive health assessments for diverse, often vulnerable, patient populations in their homes. You will review medical histories, perform physical exams, manage chronic conditions, reconcile medications, and document findings in an EHR to support value-based care programs. Collaborating with interdisciplinary teams, you'll identify gaps in care, provide education, and make referrals to improve outcomes. Responsibilities

• Perform comprehensive in-home health assessments and physical exams
• Review medical histories, medications, and social determinants of health

• Provide patient and caregiver education on conditions, medications, and self-care

• Order or recommend diagnostic tests and referrals as appropriate

• Travel within assigned regions to complete scheduled home visits
