In-Home Traveler Physician

Signify Health seeks a full-time In-Home Traveler Physician to perform comprehensive in-home health evaluations for diverse, often vulnerable populations. You will travel to patient homes, conduct detailed histories and physicals, review medications, identify care gaps, and document findings using digital tools and EHRs. Partner with care teams to support value-based care, improve outcomes, and reduce costs. Responsibilities

• Perform comprehensive in-home health evaluations, including histories, physical exams, and medication reviews
• Identify chronic condition and preventive care gaps and document findings using EHR and digital tools
• Collaborate with interdisciplinary care teams to support value-based care plans
• Communicate clearly with patients and caregivers, providing education and next-step guidance
• Travel regularly within assigned regions while maintaining high standards of clinical quality and documentation compliance
• Use data and analytics tools to inform care recommendations and quality improvement
