In-Home Traveler Nurse Practitioner or Physician Assistant

Signify Health is seeking an In-Home Traveler Nurse Practitioner or Physician Assistant to deliver comprehensive health evaluations to members in their homes. You will perform head-to-to-toe assessments, review medications, identify care gaps, and create actionable care plans using Signify's technology platform. Collaborating with interdisciplinary teams, you'll support value-based care, focusing on prevention, chronic disease management, and social determinants of health while traveling within an assigned region. Responsibilities

• Conduct comprehensive in-home health assessments for adult and senior members.
• Review medical history, medications, and social determinants to identify care gaps and risks.
• Develop and document individualized care plans in the EHR using Signify's technology platform.
• Collaborate with interdisciplinary teams to support value-based care and care coordination.
• Educate patients and caregivers on conditions, medications, and preventive care.
• Travel within an assigned region to complete scheduled member visits.
• Communicate findings to health plans and primary care providers as appropriate.
• Ensure compliance with clinical guidelines, regulatory standards, and company protocols.
