CenterWell Home Health is seeking a compassionate Home Health Registered Nurse to provide skilled in-home nursing care. In this role, you will assess patient needs, develop and implement care plans, administer treatments, and educate patients and families to promote independence and quality of life. Responsibilities
- Perform comprehensive in-home nursing assessments and ongoing evaluations of patient status
- Develop, implement, and regularly update individualized patient care plans in coordination with the physician and interdisciplinary team
- Provide skilled nursing care including wound care, medication administration, injections, IV therapies (as applicable), and chronic disease management
- Educate patients and caregivers on medications, disease processes, symptom management, safety, and self-care strategies
- Monitor and triage changes in patient condition, communicating promptly with physicians and care team to adjust treatment plans
- Accurately document all care, assessments, and communications in the electronic medical record in a timely manner
- Coordinate with therapists, social workers, and other healthcare professionals to ensure integrated, goal-directed care
- Promote patient autonomy, safety, and quality of life in the home setting through proactive care and early intervention
- Participate in case conferences, quality improvement activities, and ongoing training or education
- Adhere to all regulatory, infection control, and patient safety standards while representing Center
