VNS Health in New York City seeks a Per Diem Clinical Nurse Manager to lead and support home and community-based nursing care. In this role, you will supervise field nurses, oversee patient assessments, and ensure high-quality, patient-centered care. You will coordinate caseloads, monitor clinical documentation, and maintain regulatory compliance. Responsibilities
- Supervise and support field nurses delivering home and community-based care.
- Oversee patient assessments and development of individualized care plans.
- Monitor quality, safety, and regulatory compliance in all clinical activities.
- Coordinate caseloads, staffing, and daily assignments to meet patient needs.
- Provide clinical guidance, coaching, and performance feedback to nursing staff.
- Collaborate with physicians, therapists, and social workers on care coordination.
- Review clinical documentation and ensure accurate, timely EHR entries.
- Participate in case conferences and family meetings to align care goals.
- Support onboarding and ongoing training of clinical team members.
- Contribute to process improvements that enhance patient outcomes and experience.
