Job Description
VNS Health is seeking a Licensed Psychiatric Social Worker to provide compassionate mental health services in home and community-based settings across New York City. In this role, you will conduct psychosocial assessments, develop and implement treatment and safety plans, and deliver evidence-based counseling to adults and families. You will coordinate care with nurses, physicians, and community providers, advocate for client needs, and link clients to social services. Responsibilities
- Conduct comprehensive psychosocial and mental health assessments for patients in home and community settings.
- Develop, implement, and update individualized treatment and safety plans.
- Provide individual and family counseling using evidence-based practices.
- Respond to and manage psychiatric and psychosocial crises, coordinating emergency interventions as needed.
- Collaborate with nurses, physicians, and other care team members to ensure integrated care.
- Coordinate referrals and connect patients and families with appropriate community and social service resources.
- Advocate for patient needs and support recovery-focused, person-centered care.
- Maintain accurate, timely clinical documentation in electronic health records (EHR).
- Educate patients and caregivers on mental health conditions, treatment options, and coping strategies.
- Participate in case conferences, supervision, and ongoing professional development.
