Job Description
VNS Health in New York City seeks a Psychiatric Social Worker to provide home and community-based mental health services. In this role, you will complete psychiatric and psychosocial assessments, develop person-centered care plans, and deliver counseling, crisis intervention, and care coordination for diverse populations. You will collaborate with an interdisciplinary team to support patients and caregivers, connect them with community resources, and promote safety and recovery. Responsibilities
- Conduct comprehensive psychiatric and psychosocial assessments in home and community settings
- Develop and implement individualized treatment and care plans in collaboration with an interdisciplinary team
- Provide evidence-based counseling, crisis intervention, and support to patients and families
- Coordinate services and connect patients with community resources and support programs
- Monitor patient progress, adjust care plans, and document all care in electronic health records
- Collaborate closely with psychiatrists, nurses, and other clinicians to optimize care
- Educate patients, families, and caregivers on mental health conditions, treatment options, and coping strategies
- Participate in case conferences, team meetings, and quality improvement initiatives
- Advocate for patients' rights and access to appropriate behavioral health services
