Physical Therapist

Jefferson Regional Specialty Hospital seeks a skilled Physical Therapist to join our Healthcare & Medical Services team. Responsibilities

• Perform comprehensive physical therapy evaluations, including strength, range of motion, balance, gait, and functional mobility assessments.
• Develop and implement individualized, evidence-based treatment plans aligned with physician orders, patient goals, and best-practice guidelines.
• Provide therapeutic interventions such as manual therapy, therapeutic exercise, neuromuscular re-education, gait and balance training, and functional mobility training.
• Educate patients and families on diagnosis, treatment plans, safety precautions, use of assistive devices, and home exercise programs to support continued recovery.
• Collaborate closely with physicians, nurses, and other rehabilitation team members to coordinate care and optimize patient outcomes.
• Document evaluations, daily treatment notes, progress reports, and discharge summaries accurately and promptly in the electronic medical record system.
• Monitor patient progress, adjust treatment plans as needed, and communicate changes and recommendations to the care team and patient/family.
• Adhere to all hospital policies, regulatory standards, ethical guidelines, and infection control and safety procedures in all clinical activities.
• Participate in interdisciplinary rounds, case conferences, and discharge planning to support safe, effective transitions of care.
• Engage in continuing education, in-services, and professional development activities to maintain and advance clinical competency.
• Contribute to departmental quality improvement initiatives and help promote a culture of compassionate, patient-centered care.
