Job Description
LifeBridge Health, Inc. seeks a Clinical Pharmacist II to provide advanced pharmaceutical care across inpatient and ambulatory settings. The pharmacist will optimize medication therapy, perform clinical pharmacokinetic dosing, oversee sterile and non-sterile compounding, and support safe, cost-effective formulary use. Responsibilities include order verification, rounding with interdisciplinary teams, patient education, antimicrobial and anticoagulation stewardship, and responding to codes. The role also precepts learners, contributes to quality and safety initiatives, and supports a patient-centered, community-focused care environment.
Responsibilities
- Verify and clinically assess medication orders for safety, efficacy, and cost-effectiveness
- Provide pharmacokinetic dosing, therapeutic drug monitoring, and medication adjustments
- Participate in interdisciplinary rounds and collaborate with providers on medication therapy plans
- Oversee sterile and non-sterile compounding in compliance with USP and institutional policies
- Support antimicrobial and anticoagulation stewardship initiatives
- Educate patients and caregivers on medication use, adherence, and safety
- Respond to codes and emergencies with medication expertise and preparation
- Precept pharmacy students and residents; contribute to staff education
- Participate in quality improvement, medication safety, and formulary management projects
- Maintain accurate documentation and comply with regulatory and accreditation standards
