In-Home Nurse Practitioner or Physician Assistant

Signify Health is seeking a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for Medicare and other vulnerable populations in their homes. You will complete detailed health risk assessments, review medications, address care gaps, and educate patients on chronic disease management and prevention. Using Signify Health's technology and data tools, you'll document encounters, support value-based care, and collaborate with remote clinical teams. Responsibilities

• Conduct comprehensive in-home health risk assessments for Medicare and vulnerable populations
• Review medical histories, medications, and social determinants of health to identify care gaps
• Document findings accurately in electronic health records and Signify Health platforms
• Educate patients and caregivers on chronic disease management and preventive care
• Collaborate with Signify Health's clinical and care coordination teams to develop care plans
• Use data and technology tools to inform clinical decisions and support value-based care
• Maintain compliance with clinical guidelines, HIPAA, and quality standards
• Communicate clearly with health plans and primary care providers as needed
• Manage a flexible, part-time schedule while meeting visit and documentation expectations
• Participate in ongoing training, quality reviews, and performance improvement initiatives
