LifePoint Health is seeking a dedicated Occupational Therapist to join our Healthcare & Medical Services team. In this role, you will assess, plan, and implement individualized therapy programs to help patients regain function, independence, and quality of life. You will collaborate closely with physicians, nurses, and other therapists to deliver patient-focused, evidence-based care. Responsibilities
- Evaluate patients' functional status, range of motion, and cognitive/physical abilities
- Develop individualized, evidence-based treatment plans and goals
- Provide therapeutic interventions to improve ADLs, mobility, and functional independence
- Educate patients and families on home programs, safety, and adaptive techniques
- Collaborate with physicians, nurses, and rehab team to coordinate care
- Document assessments, progress notes, and discharge summaries in EMRRecommend and train patients in use of adaptive equipment and assistive devices
- Participate in care conferences and contribute to discharge planning
- Ensure compliance with regulatory, safety, and infection control standards
- Support quality improvement and community health initiatives within the facility
