Job Description
Essentia Health is seeking a compassionate Nursing Assistant for our Long-Term Care team. In this role, you will provide direct bedside care, assist with daily living activities, and support residents' comfort, dignity, and safety. You'll take vital signs, help with mobility and hygiene, and accurately document care in collaboration with RNs and LPNs. Responsibilities
- Provide direct personal care, including bathing, grooming, toileting, and dressing for long-term care residents
- Assist with safe transfers, mobility, and positioning using proper body mechanics and equipment
- Measure and record vital signs, intake/output, and other basic observations accurately
- Support residents with meals, hydration, and nutritional needs while monitoring for swallowing or dietary issues
- Maintain a clean, safe, and dignified environment, following infection control and safety protocols
- Document care and observations in electronic health records as directed
- Promptly report changes in resident condition to RNs or LPNs
- Participate in care plan implementation and collaborate with the interdisciplinary care team
- Provide emotional support and companionship to residents and their families
- Adhere to Essentia Health policies, promoting patient-centered care and a respectful, team-based culture
