CenterWell Home Health seeks a Medical Assistant to support personalized, in-home patient care. In this role, you'll assist nurses and therapists with clinical tasks, vital signs, basic wound care, and documentation while promoting patient autonomy and safety. You'll collaborate closely with an interdisciplinary team to deliver high-quality, compassionate care and educate patients and families on home-based health practices. Responsibilities
- Provide in-home clinical support to patients, including vital signs, basic wound care, and assistance with activities of daily living under RN or physician direction
- Prepare and maintain accurate electronic health records, documenting patient status, interventions, and changes in condition
- Assist with medication reminders and monitoring, reporting concerns to the supervising nurse or clinician
- Coordinate care with nurses, therapists, and other team members to support individualized care plans
- Educate patients and families on basic care techniques, safety, and disease management as directed
- Maintain infection control, safety, and quality standards in the home setting
- Travel between patient homes within the service area, managing schedules and visit documentation
