Home Health Registered Nurse Case Manager

CenterWell Home Health, part of Humana Inc., is seeking a Home Health Registered Nurse Case Manager to deliver compassionate, in-home care to seniors and adults with chronic conditions. In this role, you'll manage a caseload of patients, conduct comprehensive assessments, develop individualized care plans, and provide skilled nursing services such as wound care, medication management, and patient education. You'll collaborate closely with an interdisciplinary team to support recovery, manage illness, and promote independence at home. Responsibilities

• Provide comprehensive in-home nursing assessments and develop individualized care plans for adult and senior patients
• Coordinate and manage patient caseload, ensuring timely visits, follow-up, and accurate clinical documentation
• Administer medications, treatments, and wound care while monitoring patient response and condition changes
• Collaborate with physicians, therapists, and interdisciplinary team members to optimize patient outcomes
• Educate patients and caregivers on disease management, medications, and home safety to support independence
• Advocate for patient needs, arrange necessary services, and facilitate smooth transitions of care
• Ensure compliance with clinical protocols, regulatory standards, and agency policies in all care activities
• Use electronic health records and mobile tools to document care and communicate with the care team
