Home Health Licensed Practical Nurse

CenterWell Home Health, part of Humana Inc., seeks a Home Health Licensed Practical Nurse to deliver compassionate, in-home care to seniors and adults with chronic conditions. In this role, you'll provide skilled nursing services including medication administration, wound care, and monitoring of complex conditions, while documenting care using electronic health records. You'll collaborate closely with an interdisciplinary team to develop and follow care plans, educate patients and families, and support safe, independent living at home. Responsibilities

• Provide in-home nursing care to adult and senior patients, following individualized care plans
• Administer medications, treatments, and wound care per physician orders
• Monitor patient conditions, document observations, and update clinical records accurately
• Collaborate with RNs, therapists, physicians, and caregivers to coordinate care
• Educate patients and families on disease management, medications, and safety in the home
• Support safe transitions from hospital or facility to home care
• Use electronic health records and mobile tools to manage visits and documentation
• Participate in case conferences and interdisciplinary team meetings
• Follow all clinical, safety, and infection-control protocols
