Home Health Registered Nurse

CenterWell Home Health, part of Humana Inc., is seeking a Home Health Registered Nurse to deliver skilled in-home care to seniors and adults with chronic conditions. In this role, you'll perform comprehensive assessments, administer medications and treatments, and develop individualized care plans that promote safety, recovery, and independence at home. You'll educate patients and caregivers, coordinate closely with an interdisciplinary care team, and maintain accurate, timely documentation. Responsibilities

• Provide skilled in-home nursing care to seniors and adults with chronic conditions
• Complete comprehensive patient assessments and update care plans accordingly
• Administer medications, treatments, and wound care per physician orders
• Educate patients and caregivers on disease management, medications, and safety
• Coordinate care with an interdisciplinary team including therapists and physicians
• Document all visits, interventions, and patient progress accurately and timely
• Monitor patient status, report changes, and advocate for needed interventions
• Ensure compliance with clinical, safety, and regulatory standards
• Participate in case conferences and quality improvement initiatives
• Support patient independence and goals for remaining safely at home
