Home Health RN Clinical Manager

CenterWell Home Health seeks a Home Health RN Clinical Manager to oversee in-home clinical operations and ensure high-quality, patient-centered care. This role leads and mentors field nurses, coordinates interdisciplinary care plans, and monitors clinical outcomes, documentation, and regulatory compliance. The RN Clinical Manager supports staff training, schedules and case assignments, and partners closely with physicians and families to help seniors and adults with chronic conditions recover and remain safely at home.

Responsibilities

• Oversee daily clinical operations for a home health branch or territory
• Supervise, coach, and evaluate field RNs and other clinical staff
• Coordinate interdisciplinary care plans with physicians and therapists
• Ensure accurate, timely OASIS and EHR documentation and regulatory compliance
• Monitor quality metrics, patient outcomes, and service standards
• Assist with staff scheduling, caseload assignments, and visit productivity
• Support onboarding, training, and ongoing development of clinical staff
• Collaborate with patients and families to educate on disease management and safety
• Participate in case conferences and performance improvement initiatives
• Promote a patient-first, compassionate culture aligned with Center
