Home Health Clinical Manager

CenterWell Home Health seeks a Home Health Clinical Manager to lead clinical operations for our in-home care team. This role oversees care planning, staff supervision, and quality outcomes for a diverse patient population. Responsibilities include managing clinical staff, ensuring regulatory compliance, monitoring OASIS documentation, and coordinating with physicians and families to deliver safe, patient-centered care. You will drive performance improvement, support field clinicians, and foster a culture of compassion, integrity, and excellence in a collaborative, growth-focused environment.

Responsibilities

• Oversee daily clinical operations for home health services to ensure safe, high-quality patient care.
• Supervise and mentor field clinicians, providing guidance, performance feedback, and support.
• Coordinate and approve patient care plans, admissions, recertifications, and discharges.
• Monitor quality, patient outcomes, and regulatory compliance; lead corrective action and QI efforts.
• Ensure accurate and timely OASIS and clinical documentation within EMR systems.
• Collaborate with physicians, referral sources, and interdisciplinary teams to optimize care.
• Manage clinical staffing, caseload distribution, and visit utilization for efficiency.
• Support patient and family education, addressing concerns and ensuring satisfaction.
• Promote a culture of compassion, integrity, and continuous improvement.
• Participate in orientation, training, and professional development initiatives for clinical staff.
