CenterWell Home Health seeks a Home Health RN Clinical Manager to lead a team delivering high-quality, in-home nursing care. In this role, you will oversee clinical operations, ensuring accurate assessments, individualized care plans, and compliance with Medicare and regulatory standards. You will supervise and mentor field clinicians, coordinate interdisciplinary services, and champion patient safety and quality outcomes. Responsibilities
- Provide clinical leadership for home health nursing staff, ensuring safe, high-quality in-home care.
- Oversee patient assessments, care plans, and OASIS documentation for regulatory compliance.
- Supervise, mentor, and evaluate clinicians to support performance and professional growth.
- Coordinate interdisciplinary care to optimize patient outcomes and autonomy at home.
- Monitor quality metrics, patient safety, and satisfaction; implement improvement initiatives.
- Collaborate with physicians, therapists, and families to align treatment goals and services.
- Ensure adherence to Medicare, Medicaid, and agency policies and procedures.
- Support staff scheduling, caseload management, and resource allocation.
- Participate in case conferences, audits, and education programs.
